Sick of Waiting: Collaborating with OpenAI

There’s a NOT SO QUIET revolution happening in healthcare - and it isn’t starting in the hospital boardroom or a research lab. It’s starting with and driven by patients and carers who refuse to wait for the system to “get around to it.”

This is the Digital Human Cardiac Coach (DHCCGPT) - a custom-defined large language model created specifically for the heart patient and their carer. Created by a heart patient #CardiacMan and me, a Carer. Not overnight. But over twenty years.

This is a real life conversation recorded between Coach and me, where Coach is explaining the difference between the general ChatGPT and the DHCCGPT Custom Agent. Take a look.

We’ve built this not as another chatbot, but as a context-rich digital human - one that understands the patient’s lived journey after heart surgery. And the carer's often ignored journey. It doesn’t replace a doctor, although many should be replaced. It replaces confusion. It explains what the clinical system assumes people already know - how to go home safely, how to move again, how to manage medications, how to support recovery as a carer.

🧠 So how is DHCCGPT different from the general ChatGPT?

Because it’s context-aware. ChatGPT is a vast general model - extraordinary, yes, but encyclopaedic by design.

Listen to ChatGPT itself explain the difference.

DHCCGPT, by contrast, is a custom-defined model with a structured cardiac knowledge base - over 400 pages across 15 themed modules, and thousands of plain language entries - integrating guided conversations, safety prompts, and carer dialogue.

So when someone asks “Can I go for a walk?” or “What does my carer need to know?”, it doesn’t answer generically. It answers as Coach, a digital human designed for that recovery journey. The general ChatGPT does not have the context to answer these questions - and it says so itself.

And yet the DHCCGPT still draws on ChatGPT’s broader intelligence - the best of both worlds:

  • The depth and empathy of the DHCC corpus, and

  • The power, fluency and adaptability of OpenAI’s global platform.

Working with OpenAI on this has been an extraordinary experience - collaborative, creative, and technically seamless. It shows what’s possible when applied research, human insight, and next-generation AI come together.

💥 Why this changes the game

Because this is patients and carers taking control of the conversation. No forms. No portals. No waiting rooms. And no EHR.

Just clear, consistent, human-centred conversations - anywhere in the world, in plain language.

And the economic impact? Let’s just say this: every hospital readmission prevented, every avoided duplication of “rehab paperwork,” every hour saved in clinical explanation time - all compound. The ripple effect on health systems is enormous. We've done the numbers.

The DHCCGPT is now being tested globally. Early feedback? Transformative.

This isn’t just technology - it’s agency. We’ll be sharing more videos and insights over the coming weeks. Real conversations with Coach, real reactions from around the world.

The future of cardiac recovery is conversational. And it has just begun.
